Outpatient Services and Treatments Include:

A Diagnostic/Comprehensive Clinical Assessment is an intensive clinical and functional face-to-face evaluation of a beneficiary’s mental health, intellectual and developmental disability, or substance use condition. The assessment results in the issuance of a report with a recommendation regarding whether the beneficiary meets target population criteria and includes a recommendation for services that provides the basis for the development of the Treatment Plan.

Outpatient Therapy is a basic level service that is designed to work with individuals and/or families to address identified mental health, substance use or developmental disability diagnosis or a combination along with medical comorbidities. This service is aimed at helping individuals achieve emotional wellness when they are experiencing or dealing with emotional issues, relationship issues within and outside the family. Our team works with clients to truly understand their issue and develop strategies to improve specific situations. As part of this service, Lone Star State Healthis able to offer trauma focused cognitive behavioral treatment by rostered clinicians.

Medication Management and Evaluation is a service that is offered in the array of services where a client is evaluated by a licensed medical staff to see if the client needs medications to reduce or eliminate symptomology of a diagnosed mental health, substance use or developmental disability. All medications shall be based on a proper medical order and prescribed by a qualified medical professional. Clients will receive a prescription from medical staff to be filled by a pharmacy of the client’s choice.

Lone Star State Health is proud to connect clients with experienced Trauma Focused CBT clinicians. Trauma comes in many forms and can include sexual abuse, rape, physical abuse, neglect, witnessing or surviving domestic violence,  witnessing or involvement in a car accident, or witnessing or involvement in a violent crime. Our TF-CBT is a hybrid treatment model that incorporates various trauma-sensitive interventions, cognitive behavioral principles, attachment theory, developmental neurobiology, along with family, empowerment, and humanistic therapy.  TF-CBT techniques are individualized for youth ages 3-18 years old and their circumstances and maintain therapeutic relationships with both the child and caretaker. TF-CBT evidence based treatment can be used with children and adolescents who have experienced a traumatic life event.  TF-CBT includes individual sessions for both the child and the caretaker, as well as parent-child conjoint sessions.

Enhanced Services Include:

Services are designed to address the needs of children and adolescents, who due to serious and chronic symptoms of emotional, behavioral, and/or substance use disorders, are unstable in the home and community without intensive interventions.  This limited time, home-based mental health service is a team approach designed to meet the unique needs of each child and family through crisis management, intensive case management, counseling, family therapy, and skills training. The primary goal of the services is to prevent out-of-home placement or provide transition services from out-of-home placement back into the home and aims to help families solve problems within the context of their families rather than through placement outside of the home. Services are based on the belief that the family is the most powerful social institution and that families should be supported and maintained whenever possible. Our Intensive In-Home Services staff is available to families for crisis response 24 hours a day, 7 days a week. 

This outpatient service is offered at our licensed facility and provides mental health treatment for children and adolescents who are experiencing mental, emotional and/or behavioral disturbance that prevent the child from functioning appropriately in the school setting.  The goal of Day Treatment aims to deliver educational services in conjunction with mental health services to meet student needs while accessing their educational curriculum. The program offers an environment that addresses the unique needs of each child while utilizing the benefits of individual, group and family based treatment approaches. Daily therapeutic interventions may include strengthening coping skills, socialization skills, communication skills, problem-solving and conflict resolution to reduce symptoms, emotional and/or behaviors problems.

Day Treatment service delivery is part of a multidisciplinary team with family, students, educators, and other mental health providers to ensure success in achieving goals set out by the student’s educational plan and Person-Centered Plan (PCP). Students are taught positive problem-solving skills through Cognitive Behavioral Therapy (CBT) and appropriate social interaction skills by interacting with and role-modeling by all staff.

This service is designed to help adults with psychiatric disabilities. PSR focuses on skill and resource development related to life in the community with the goal of increasing the participant’s ability to manage their illness, live independently with as little professional intervention as possible, and to participate in community opportunities related to functional, social, educational and vocational goals.

Adults, Children, and Adolescents.

At Lone Star State Health, we offer carefully coordinated and personalized support services for individuals with intellectual disabilities, developmental delays and developmental disabilities. Our service coordination team works with each consumer, and her/his family to understand their goals and objectives and craft an ITP (Individual Treatment Plan) to meet them.

Our Direct Support Professionals offer a wide array of services in home, at school, at work, and throughout life including:

Community Living and Support is an individualized service that enables the waiver beneficiary to live successfully in his/her own home, the home of his/her family or natural supports and be an active member of his/her community. The intended outcome of the service is to increase or maintain the person’s life skills or provide the supervision needed to empower the person to live in the home of his/her family or natural supports, maximize his or her self-sufficiency, increase self- determination and enhance the person’s opportunity to have full membership in his/her community. Community Living and Support enables the person to learn new skills, practice and/or improve existing skills. Areas of skill acquisition are: interpersonal, independent living, community living, self-care, and self-determination.

Respite services provide periodic or scheduled support and relief to the primary caregiver(s) from the responsibility and stress of caring for the individual. This service enables the primary caregiver to meet or participate in planned or emergency events, and to have planned time for him/her and/or family members. This service also enables the individual to receive periodic support and relief from the primary caregiver(s) at his/her choice.

Supported Employment Services provide assistance with choosing, acquiring, and maintaining a job for beneficiaries ages 16 and older for whom competitive employment has not been achieved and /or has been interrupted or intermittent. The intent of Initial Supported Employment is to assist individuals with developing skills to seek, obtain and maintain competitive employment or develop and operate a micro-enterprise. The employment positions are found based on individual preferences, strengths, and experiences. Job finding is not based on a pool of jobs that are available or set aside specifically for individuals with disabilities. The transition to long-term supported employment should occur within one year of successful competitive employment, at this time it is expected that staff time will reduce as the individual becomes more independent in her/his job duties.

Developmental Therapy is a developmental disability service that includes individually designed instruction, training or functional developmental intervention activities based on the assessment of, and unique strengths and needs of the individual child or adult. It is designed to support the individual in the acquisition of skills that the recipient has not gained during the developmental stages of life, and is not likely to develop without additional training and support.

Our Personal Assistance services focus on the strengths of individuals with developmental disabilities, with the goal of increasing the client’s ability to engage in community opportunities related to functional, social, educational and vocational goals. This support service is designed to provide aid in a manner that opens the door to participation in activities and interactions from which the client would otherwise face limitations or be excluded from because of his/her disability or disabilities. Depending on a client’s individual needs, the service could include assistance in personal or regular living activities in the home, support in skill development, or support and accompaniment of the client in regular community activities or in a specialized treatment, habilitation or rehabilitation programs.
